Yes, this Four-Light Pendant comes supplied with two (2) 12-inch & two (2) 6-inch adjustable down rods. An overall height: 55-inch.

Can this be hung on a sloped 12 foot ceiling?  Does it require extras?

The maximum hanging height of this One-Light Pendant 55" inch and it comes supplied with Slope ceiling kit to hang on sloped ceiling.
