When the screw is inserted through the plate and into the vertical pipe, the screw head is not flush with the deck surface. That makes the whole plate wobble on the deck.
Correct. This umbrella clamp is not flush and not intended to be attached to a completely flat surface like a deck. It is used with tables that have a brace/crossbar at the bottom of the table. In place of a standard umbrella stand.

How is the pipe attached to the base plate?
There is no hardware used to attach the base to the post. The bottom of the post has a threaded extension used to attach the base.
